CLAIMING YOUR CERTIFICATE

Attendees will complete a post-symposium online evaluation to claim their certificates, which will be emailed in a secure document approximately two weeks after submission. You will receive more information about this process at the forum.

CME ACCREDITATION & DESIGNATION 

NUHCS-NCIS Thrombosis Forum 2017 is accredited by the Singapore Medical Council (SMC), Singapore Nursing Board (SNB) and Singapore Pharmacy Council (SPC) to provide continuing medical/professional education for clinicians, nurses and pharmacists alike.

For Clinicians
The SMC has approved the forum for a maximum of 4 CME points. Clinicians should claim only the credit commensurate with the extent of their participation in the activity.

For Nurses
The SNB has approved the forum for a maximum of 4 CPE points. Nurses should claim only the credit commensurate with the extent of their participation in the activity.
* SNB has renamed its point system as Continuing Professional Education (CPE).

For Pharmacists
The SPC has approved the forum for a maximum of 4 CPE points. Pharmacists should claim only the credit commensurate with the extent of their participation in the activity.
